You have been identified as a potential Eligible Investor because of your transactions with Ross Sinclaire between January 2007 and December 2012 (the “Relevant Period”); therefore, you may be eligible to receive compensation in accordance with the provisions of the Plan of Distribution approved by the Commission.

The Certification Form must be submitted no later than June 8, 2019.

The amount available for distribution is $959,156.13 which will earn interest, to be distributed, after deduction of Court-approved attorneys’ fees and expenses, settlement administration costs, and any applicable Taxes (the “Net Settlement Fund”) among all Eligible Investors who submit valid Certification Forms and are found by the Court to be entitled to a distribution from the Net Settlement Fund.

If you believe you are entitled to act on the deceased or incapacitated Eligible Investor’s behalf please provide a copy of any death certificate and a letter detailing who you are and your relationship to the putative Eligible Investor’s along with the signed Certification Form.

If you desire an acknowledgment of receipt of your Certification form, please send it via Certified Mail, Return Receipt Requested, or its equivalent. You will bear all risks of delay or non-delivery of your form.
